Introduction

Soma contains carisoprodol, a centrally acting skeletal muscle relaxant used to relieve discomfort associated with acute, painful musculoskeletal conditions. It works by interrupting neuronal communication within the brain and spinal cord, resulting in muscle relaxation and pain reduction.

Carisoprodol is metabolized to meprobamate, a substance with anxiolytic and sedative properties, which contributes to its therapeutic—and potential dependence—effects. Because of these effects, Soma is classified as a Schedule IV controlled substance in the United States.

Soma has been available since the late 1950s. It is widely used in the U.S. and in some other countries, but availability differs internationally due to concerns about misuse. Generic carisoprodol tablets are common; they typically come in 350 mg strength.

Used for

Soma (carisoprodol) is approved for the short-term treatment of acute musculoskeletal pain in adults. It may be used when patients experience:

  • Painful muscle spasms
  • Musculoskeletal strain or injury
  • Back pain
  • Acute painful orthopedic conditions

Use is generally recommended only for short durations (2–3 weeks) because many musculoskeletal conditions are self-limiting and because the medication carries risk of dependence.

Important information

Soma may cause serious side effects, including:

  • Dependence, misuse, and abuse: Risk increases with prolonged use, high doses, or a history of substance use disorders.
  • Sedation and respiratory depression: Especially dangerous when combined with CNS depressants (e.g., opioids, benzodiazepines, alcohol).
  • Impaired ability to operate machinery or drive: Soma can cause drowsiness and decreased motor coordination.
  • Withdrawal symptoms: Abrupt discontinuation after prolonged use may cause anxiety, tremors, hallucinations, and insomnia.
  • Risk of seizures: Has been reported in individuals misusing high doses or combining Soma with other substances.

Who should avoid?

Do not take Soma if:

  • You are allergic to carisoprodol, meprobamate, or related compounds. Symptoms include rash, breathing difficulty, or swelling of the lips, face, throat, or tongue.
  • You have acute intermittent porphyria.
  • You have previously experienced idiosyncratic reactions (e.g., agitation, confusion) to carisoprodol or meprobamate.
  • You are under 16 years of age (safety and efficacy not established).

Precautions

Before using Soma, tell your healthcare provider if you:

  • Have liver or kidney impairment (metabolism and elimination may be affected).
  • Have a history of substance use disorder.
  • Take opioids, benzodiazepines, alcohol, or other sedatives.
  • Have had seizures or conditions that lower seizure threshold.
  • Have used Soma or similar medications long term.

How should you take Soma?

(General information only — always follow your prescriber’s instructions.)

  • Take Soma exactly as prescribed.
  • Treatment should be as short as possible, usually for 2–3 weeks.
  • Take tablets with or without food.
  • Do not increase your dose or frequency on your own.
  • If discontinuation is needed after prolonged use, your clinician may recommend gradual tapering to reduce withdrawal symptoms.

If you miss a dose:

Skip the missed dose if it is almost time for the next one.

Do not take two doses at once.

Overdose:

Seek emergency medical attention immediately. Symptoms may include:

  • Severe drowsiness or confusion
  • Blurred vision
  • Coordination problems and unsteady movements
  • Low blood pressure
  • Trouble breathing
  • Seizures
  • Coma

What should you avoid while taking Soma?

  • Alcohol — increases sedation and risk of breathing problems.
  • Opioids or other sedatives, unless specifically directed by a doctor.
  • Driving or using machinery until you know how the medication affects you.
  • Activities requiring full alertness (Soma can impair reaction time for several hours).

Dosing information

(For reference only — follow your healthcare provider’s prescribed dose.)

  • The typical adult tablet strength is 350 mg, taken up to several times daily as directed by a clinician.
  • Use is short-term only (generally ≤ 2–3 weeks).
  • Older adults or people with liver/kidney impairment may require lower doses due to increased sensitivity.

Interactions

Tell your doctor about all medications and supplements you take.

Soma may increase the effects of:

  • Opioids
  • Benzodiazepines
  • Sleep medications
  • Alcohol
  • Sedating antihistamines
  • Other central nervous system depressants

Medications that may increase Soma side effects:

  • CYP2C19 inhibitors (e.g., omeprazole, fluvoxamine) — may raise carisoprodol levels
  • Other sedative/hypnotic agents

Medications that may reduce Soma’s effectiveness:

  • CYP2C19 inducers (e.g., rifampin) — may lower carisoprodol levels

Pregnancy and breastfeeding

  • Soma should be used during pregnancy only if clearly needed.
  • Carisoprodol and meprobamate cross the placenta; neonatal sedation or withdrawal may occur.
  • Soma is not recommended while breastfeeding because both carisoprodol and meprobamate pass into breast milk and may cause sedation or feeding difficulties in infants.

Storage

  • Store at room temperature, away from moisture and heat.
  • Keep in original packaging, out of the reach of children.
  • Store securely due to misuse potential.
